You may elect voluntary term life insurance for yourself and your dependents through convenient payroll deduction to supplement the district-paid basic life benefit. The voluntary life plan also includes AD&D coverage.

Beneficiary Information: Your beneficiary is the person(s) who will receive your life insurance benefits in the event of your death. Your beneficiary can be one person or multiple people, charitable institutions, or your estate. Once named, your beneficiary remains on file until you make a change. If your family situation changes, please review your beneficiary on file and make updates if needed. If you don’t name a beneficiary, your life insurance benefits will automatically go to your estate. You are required to designate your beneficiary during your enrollment, but you can change it at any time.

Voluntary Life and AD&D Insurance Options

Employee
$10,000 increments up to the lesser of 5 times your earnings or $500,000

Spouse
Premiums based on employee's age
$5,000 increments up to the lesser of 100% of the amount of employee coverage or $250,000

Child(ren) age 6 months to 26
$10,000
The benefit for children age birth to 6 months is $1,000.

Evidence of Insurability (EOI) for Voluntary Life & AD&D Insurance


As a new hire, you are able to elect up to the Guarantee Issue of $200,000 for yourself, $50,000 for your spouse, and $10,000 for your child(ren) with no health questions. Should you wish to elect an amount that exceeds the Guarantee Issue, an Evidence of Insurability Form is required. You may obtain an EOI Form from the Resources page or by calling the Benefits Service Center. To apply, simply complete the form and submit it to Unum for review. You will not be deducted for your pending amount unless / until you are approved. If you elect coverage effective July 1, 2025, regardless of the amount, you may increase your future elections at Open Enrollment up to the Guarantee Issue with no health questions.

Coverage Continuation Options


In the event of employment separation, you may port or convert your voluntary life insurance coverage. Portability provides you with an individual term policy which terminates at age 75, and portability must be elected prior to age 70.  Conversion provides an individual whole life insurance policy.  For coverage continuation options, portability or conversion must be elected within 31 days of your employment separation, and premium payments are remitted directly to Unum.  Contact Unum to exercise these options.
